The head of the Assisted Reproduction Center (PMA) at San Raffaele described the human error that occurred, noting that the wrong line was read, which led to the implantation of the incorrect embryo.

According to reports, the likelihood of a similar error is 1 in 15,000. A change in the tracking system is being considered to prevent such incidents from happening in the future.
